Grant Application Checklist

Please print out the checklist below if you are submitting your grant application by mail or email.

Grant Applications and Required Document Checklist:

I.    One page (maximum) cover letter

II.  A Two page (maximum) grant proposal explaining:

  1. Purpose of the grant request
  2. Community need grant will address
  3. How program fulfills our Foundation's mission
  4. Implementation plan
  5. Goals for grant
  6. Any measurable outcomes anticipated
  7. Mission or goals
  8. Any management challenges, plans or other pertinent information.
  9. If you show a deficit on financial document that you are submitting to us (whether it’s your program, organizational, 990, financial statements or last quarter revenue & expense report) please provide a brief written explanation as to why you experienced the deficit and if still applicable, when you are expecting to get out of the deficit.

III.  Financial Information for Grant Period

  • Budget for $25,000 grant amount
  • 2012 Budget for program or project
  • 2012 Budget for overall organization

IV.   Historical Financial Information:

  • Itemization of last quarter revenue and expenses
  • 2011 financial statements, audited preferred; not required
  • 2010 and 2011 990
